This tiny gold lamella was rolled and very likely kept in a small tube (see an example kept at the british Museum here: https://www.britishmuseum.org/collection/object/H_1981-0201-30). The incantation reads:

The god of Abraam, the god of Eisak, the god of Iakôb, the god of us:Deliver Aurelia from every evil spiritand from every epileptic fit and seizure!
I implore you, Lord Iaô, Sabaôth, Elôaion, Ouriêl, Michaêl, Raphaêl, Gabriêl, Saraêl, Rasochêl, Ablanathanalba, Abrasax, xxxxxx nnnnnn ô aa iiiiiiiiii x o yyyyyyy ao ooooooo ônô (4 symbols) Sesengenbarpharangês:Protect!
Ippho, Io Erbêth (magic signs): protect Aurelia from every seizure, from every seizure, Iaô, Ieôu, Iêô Lammô Iaô Charakoôpou Sesengenbarphrangês, Iaô, Aeeyyai, Iêou, Iaô, Sabaôth, Adônaie, Êlêlêth, Iakô!
